Benefit Plan Summary

 

Medical:  We offer two plans.  The 1-2-3 Plan may be supplemented by a Flexible Spending Account.  The High Deductible Plan may be combined with an individual Health Savings Account.

 

Dependent Care Coverage: Flexible Spending Account.

 

Dental: Covers preventive, basic and major services.

 

Vision: Covers vision exams, frames or lenses.

 

Life and AD&D:  One tine annual salary with double benefit if you should die in an accident.

 

Optional Life Insurance: Up to five times your annual salary.

 

Short-Term Disability Insurance:  Short-term disability income benefits up to 26 weeks.

 

Paid Time Off (PTO):  Employees accrue PTO after 90 days of service,

 

Personal Days:  Two per year

 

Major Medical Time:  Sick time accrues per pay period with a 200 hour cap.

 

Retirement Plan:  We offer a 403B plan with a discretionary match approved annually by our Board of Directors.

Pretax Deductions:  Reduces your taxable income.

 

Pay Dates:  Bi-monthly payroll with 24 pay dates per year.
